Client: Atlantic County Federally Qualified Health Center 

Problem(s):

Health center needed resources to meet government-mandated health quality metrics.

Quality Improvement Measures Implemented:

Assisted health partners in creating quality dashboards relevant to underserved patients and under-resourced clinic. Updated processes, implemented training for patient care of people with Alzheimer’s Disease, Parkinson’s Disease, epilepsy, stroke, multiple sclerosis, migraine, spinal cord injury and paralysis, as well as for medical conditions that cause neurological problems.

Results:

  • Annual goal for quality metrics was exceeded and regulatory requirements were satisfied.
  • The health center improved access and adherence to recommended neurological therapies.
  • Disenfranchised patients became more engaged with their own neurological care process.
