Client: Geisinger Health System Member: Neurosciences Comprehensive Stroke Program
Problem(s):
A regional health system serving 1,000,000 customers in the mid-Atlantic, southern New Jersey region required timely (local) treatment to improve quality outcomes for the stroke population to obtain Joint Commission comprehensive stroke center status.
Program Developed:
- Neural Network Health leaders assisted in deployment of collaborative care across business units and transitional care services for acute and post-acute stroke patients treated in the Neurosciences Center.
- The project encompassed training staff, deploying quality improvement measures and providing a continuum of care as a basis for optimal quality metrics and standardized procedures for stroke care.
Business Results:
The medical center successfully met business requirements while attaining status as a qualified comprehensive state stroke center Success was achieved in maintaining all program compliance standards.
The Neuroscience program received US News and World Report national ranking for neuroscience and stroke care.
